1. Cause:

First, we need to know the reason why you have low testosterone (hypogonadism). This is done by obtaining lab work and a thorough history and physical exam.

A. One cause may be primary hypogonadism where the brain (pituitary gland) is sending the right hormones (luteinizing hormone) to the testicles. However, the testicles fail to produce adequate amounts of testosterone. This may be caused by aging, testicular injury, infection, type 2 diabetes, and genetic disorders.

B. Another possible cause is secondary hypogonadism. This is a condition where the pituitary gland fails to send adequate amounts of hormones (luteinizing hormone) to the testicles. This suboptimal hormone cascade will cause a lower production of testosterone from the testis. This may be caused by aging, repeated head injury (concussions), and pituitary adenomas.

C. A third possible cause for hypogonadism is a combination of the two. The pituitary gland and testicles fail to produce adequate amounts of hormones (luteinizing hormone, testosterone) that the body requires. A few causes could be aging, obesity, exposure to hormone disruptions in the environment, infections, previous steroid abuse, and infection.

2. Low Testosterone Treatment Options:

A. Testosterone Injections – Injected two or more times a week. This will produce optimal blood levels with lower peaks and valleys.

B. Testosterone Creams or Gels – Applied daily. This delivery method is effective in creating optimal testosterone levels.

3. Follow Up

Within 10 weeks after initiating TOT additional lab work will be performed. This is a critical part of the process that lets us know if your testosterone dosage is within the optimal range for your specific needs. We will also schedule a follow-up consultation to review your lab work, diet, and exercise regime, and if needed, adjust your testosterone dose.
